** Students must attach a copy of their student ID, Residents and Trainees a signed certificate from their department head or supervisor. Age limit to 40 years.

^§ Registration fee for all Accompanying persons, no matter from which area, do not include attendance to any of the Conference Scientific Sessions. It include: participation in Social Events (Opening Ceremony, Welcome Cocktail and Closing Ceremony), and 3-day Veronacard. Gala Dinner is not included.

^≠ Please note that you have to decide at the time of online registration whether you want to go to the Gala Dinner on Friday evening, October 4 because number of tickets is limited and there will be no on-site ticket sale.

HOW TO REGISTER

In order to register for the Conference, you are kindly requested to fill in the attached registration form, to sign and send it by fax to COGEST M. & C. (0039 045 597265) together with the registration fee payment. COGEST M. & C. will send you an official receipt for the payment and further organizing information.

You can choose to pay by:

• Bank transfer to be made to COGEST M. & C., IBAN IT49H0200811705000005233961, BIC SWIFT UNCRITM1M03, on UNICREDIT BANCA AGENCY Piazza Bra 26/e, Verona Italy specifying the title of the Conference and the surname of the person you are making the payment for; copy of the bank receipt must be faxed (0039 045 597265) with the form
• Credit card, filling in the appropriate box in the form (AMERICAN EXPRESS, DINERS, MASTERCARD/EUROCARD, VISA) Other forms of payment will not be accepted (postal order, cash, etc.)

REGISTRATION CANCELLATION AND REFUND POLICY

Notification of cancellation before September 3, 2013 will result in 70% refund of the total amount paid. Refunds will be made after the Conference.
Notification of cancellation after this date will result in no refund.
